Jeffrey Polka

Jeffrey Scott Polka born October 14, 1962 passed away on Tuesday, April 11, 2023.  Our loss is heavens gain, and Jeff will forever be in our hearts. Jeffrey is survived by his wife, Kimberly Polka (née Kapelski) daughters  Erica (Lance Ingram) Polka and Samantha (William) Harris; his beloved grandsons Jayden Christopher Ingram, Logan Scott Rell and William Jeffrey Harris;  siblings Ellen (Richard) Heacock, Lisa (Steven) Moore, Mark Polka; nephews Jacob Heacock, Joshua (Nicole) Heacock, and Ian Moore.  Jeffrey was preceded in death by his daughter,  Kayla Anne Polka, parents Richard and JoAnne Polka, nephew, Ryan A. Moore and niece Emilee Paige Heacock.
