John “Jack” Kozup passed away on Monday, October 8, 2012. Beloved husband of Sally Kozup (nee Davis) for 47 years. Loving father to Jill (Steve) Metherd and Gina (Tom) Poure. Devoted Grandfather to Ian, Olivia, Emily, Nick and Cody, and step-grandfather to Gina, Natalia and Sophia.

Funeral service to be held at Good Shepherd Catholic Church (Hillsboro) at 10:30am Saturday, October 13. Reception immediately following in school cafeteria. In lieu of flowers, memorial contributions can be made to St. Anthony’s Hospital Charitable Foundation.
